Vermont Worksite Wellness Award Application

The Governor’s Council on Physical Fitness and Sports 2009 Worksite Wellness Awards recognize Vermont employers who promote a work environment that encourages and enables their employees to improve their overall health and well-being.    

Completing the on-line application is the first step in the 2009 awards application process. Click here to begin:

All on-line applications are due by August 31, 2009.  You can re-enter your application to make changes up to that date.  Just click on the above link each time you want to enter your application.
 
Any Vermont worksite, regardless of size is encouraged to apply.  The on-line application will collect information regarding your wellness interventions and provides ample space for you to show-case your program.

This year’s awards will be presented by Governor James Douglas at the 15th Annual Worksite Wellness Awards Ceremony, on December 1, at The Capitol Plaza, Montpelier, Vermont from 8:30-12:30.  The morning event will include a national speaker, worksite resources and exhibits, networking opportunities and a best practice panel of 2009 worksite wellness program recipients.  Continental breakfast will be served.  There is no cost to attend this event for applicants.
